Apollo edocTM is an appointment scheduler to
facilitate seeking, confirming and fulfilling appointments for doctor
consultations, health checks and diagnostic services at any of the Apollo
Hospitals across the group through the website or by calling the dedicated
Apollo edoc National telephone number - 6600 1066.

| 2. How do I book an appointment for myself/family
members/friends with a doctor at Apollo Hospitals? |
Please log on to www.apolloedoc.com, choose
your location and follow simple instructions to book an appointment with your
doctor. You may also call the National number 6600 1066 to book an appointment.
For any Apollo hospital in Andhra Pradesh, Karnataka, Tamil Nadu and New Delhi
add 040, 080, 044 and 011 respectively. Our customer care support team will
take care of your appointment. Telephonic booking facility is available from 8
am to 8 pm. You may book your appointment online minimum 2 days prior or call
the National telephone number 1 hr prior to required slot. Appointments can be
booked subject to availability of slot.

| 3. Can I cancel or reschedule my appointment? |
Once you book an appointment, you will be
provided with an Apollo edoc ID, identification no and password. You may log in
with the ID and password, and manage your appointments. You can cancel or
reschedule your appointment with the same or a different doctor depending on
the availability of doctor slot on the chosen date.

| 4. How can I make sure that my appointment is
confirmed? |
Once you book an appointment, you will be
mailed a confirmation of the same, which will have all the necessary details of
your appointment (appointment date, time, specialty and doctor's name). You
will also receive a sms provided you have given your mobile number. You will
receive a reminder sms a day before your appointment. If, for any reason, your
doctor is not available on that particular day, our customer care team will
intimate you by phone or E-mail prior to the appointment, and reschedule your
appointment. In case of emergency, please approach our Emergency department.

| 5. Is an E-mail -ID compulsory for booking on Apollo
edoc? |
E-mail ID is mandatory for booking through the
website. If you do not have an E-mail ID, you can give a family member's ID.
The purpose of providing an E-mail ID is for easy communication that may be
required from time to time. It is always advisable to provide an E-mail ID.
However, for telephonic booking, E-mail ID is not mandatory.

| 6. Do I need to fill the details again when I book an
appointment the next time? |
You do not need to provide your details again
when you book an appointment the next time. With the login ID and password, you
can book another appointment for your self or any other family member. But you
have to fill the patient details if he/she is new to Apollo edoc.

| 7. Which are the cities where this facility is
currently available? |
Currently this facility is available at
Hyderabad, Chennai, New Delhi and Bangalore. This facility will be introduced
in all other cities, wherever there is an Apollo Hospital.

| 8. Can I pay online? |
Currently, the online payment facility is not
available. You have to pay your bill amount at the respective hospital. The
online payment facility will be available soon.

| 9. Do you have a separate delivery channel at the
hospital for Apollo edoc? |
There is no separate delivery channel for
Apollo edoc. It is integrated with the general operations of the hospital.

| 10. What do I do when I reach the hospital? |
Please approach the reception/ Apollo edoc
help desk regarding your appointment. The respective staff will guide you to
the consultant. We request you to reach the hospital 15 minutes prior to your
appointment for completing the registration process. You have to pay
registration fee in addition to the consultation fee. The registration number
is valid for a period of 10 years. Should you visit again, do not pay anything
for registration; simply produce your UHID card.
